Administratively, we can aid with letter writing, completion of forms, and liaising with specialist support organizations and doctors GPs, hospital appointments.
We can provide reablement service to customers who have just been discharged from hospital or are otherwise entering the care system following a crisis. Reablement includes escorting home from hospital, preparation of the home environment, and rehabilitation support such as providing personal care, help with daily living activities and other practical tasks. Provision usually last for up to six weeks to encourage service users to develop the confidence and skills to carry out these activities themselves and continue to live at home.
Encouraging and supporting hobbies and interests at home or in the local community, encouraging exercise such as walking or a dance class, arranging and escorting people on social occasions or prayer meetings, escorting people on visits to special places and local attractions, escorting to day centres or other community day activities, escorting people on doctors’ appointments, and keeping and managing a diary.
